o Start Date: May 2015 - ASAP
o Location: Nacogdoches, Lufkin and Beaumont area
o Rate of Pay: $26.00 to $32.00 (depending on experience)
o Type: Traveling Nurses
Job Description:
o Participate in training conducted by the client, which will address the specific duties, expectations, and necessary materials/supplies.
o Order and store only approved materials and supplies from our client and the Texas Department of State Health Services (DSHS).
o No later than 2 weeks prior, contact providers to schedule a date and time and explain the purpose and composition for the VFC compliance site visit.
o Inform ou client, via email to of all scheduled/rescheduled site visits and special confirmation letter requests.
o Confirm the appointment 1-2 days prior to the scheduled visit. Ensure VFC compliance site review readiness.
o Arrive at the designated provider site according to the scheduled time with appropriate work materials.
o Complete, and check for accuracy, prior to leaving the facility and submission to our client:
o Electronic site survey tool through an environmental assessment, staff interviews, client interviews, and observation.
o For qualified providers, record review for immunization status using the CoCASA software.
o TVFC Site Assessment Summary form (SASF), including provider initials and signature.
o Schedule and complete Reminder/Recall training for qualified providers.
o Obtain accurate and completed TVFC Provider Enrollment Form (REF) before leaving the facility and submission to TMF. Providers must use the CDC approved enrollment form.
o Provide feedback to the provider's staff:
o Emphasize proper vaccine storage and handling practices, proper eligibility screening, and ensure that TVFC providers monitor vaccine expiration dates and identify/report expired vaccines.
o Reinforce strengths and areas where providers are doing well with their vaccine programs.
o Identify and discuss possible areas for improvement in provider's vaccine program to include areas of non-compliance needing additional attention (e.g., policy change, training, education, etc.).Provide education and technical assistance as needed (based on findings from the assessment).Provide and/or review resources specific to the needs of the provider.
o Provide the name and contact information for the local or regional health department immunization contact and the closest FQHC/RHC, if applicable.
o All VFC compliance site-visit materials must be submitted to the client as soon as the visit is completed and received no later than 5-7 calendar days from the date of visit. If, for any reason, it is known that the VFC compliance site-visit materials will not be received by the 7th day, TMF must be notified immediately.
o VFC Compliance Site-Visit materials will include:
o Uploaded files/documents to the website portal (or flash/jump drive when portal not available) with completed (note- upload to the portal prior to sending in your FedEx packets):
o CoCASA data (when appropriate)
o DSHS Immunization QA Site Survey Tool (including the Header, Immunization Record Review (IRRT) and Summary)
o Time/Expense Worksheet (TEW)Ralso email a copy to Staffing Agency
o Completed Review Report Form (RRF) Via FedEx to TMF
o Completed and signed SASF
o Completed and signed REF
o Any other relevant materials
Qualifications and Education:
License, Certification, or Registration
MUST HAVE Licensed Vocational Nurse (LVN) OR Registered Nurse (RN) with current Texas license
High school diploma or equivalent

Requirements and Expectations:
o Exhibit professional demeanor and dress.
o Utilize a customer-service and education-oriented approach rather than an audit approach in conducting site reviews.
o Comply with confidentiality requirements of all patient medical records and other patient information in accordance with applicable law and generally recognized ethical standards, including the regulations promulgated under Health Insurance Portability and Accountability Act, 45 CFR Parts 160 and 164.
o Understand that conducting TVFC compliance site visits and submitting acceptable VFC compliance site visit reports requires continual learning and development. For this reason, TMF will provide reviewers with up-to-date TVFC requirements. It is expected reviewers will demonstrate proficiency in all existing and updated TVFC requirements.
o Staff will be providing ongoing feedback to reviewers. A professional response is expected and, when indicated, reviewers will use the feedback to improve performance.
o Contact our client's staff at any time for clarification and assistance.
o Must provide own transportation to and from assigned work locations.
o Safeguard all of the client's equipment, to include:
o All hardware, software and air cards (if applicable),Laptop computer is for VFC project use only , Miscellaneous client-issued materials, hardware and software.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
