Acts as a liaison between the Agency, and the population it serves.
By working in the community, the case manager should provide knowledge of the agency and community resources.
In some cases, the case manager must perform direct services to secure the skills, knowledge, motivation, opportunities, and services for low-income families to become self-sufficient and to work toward removing structural barriers to self-sufficiency.
Qualifications and Requirements:
Minimum of certificate or associate degree in Human Service, or a related field, however a bachelor's degree is preferred.
Applicant must have organizational, communication, and community health skills.
Applicant must be able to work with minimal supervision.
Prior to starting employment, a criminal background check must be completed.

Essential Functions:
General Duties:

Responsible for going through the chain of command by discussing problems with immediate supervisor and following all personnel policies.

Attends training sessions as appropriate including out of town, evening, and weekend training.

Responsible for recording dates and topics of all training received.
6.
Responsible for maintaining confidentiality regarding families and agency business.
7.
Completes other duties as requested by supervisor.
8.
Document time in EWS twice a day.
9.
Attends work according to schedule established by supervisor.
Service Area Specific Duties:
General Responsibilities Coordinates and promotes all Greater East Texas Community Action Programs in the designated area and determines eligibility for energy related and refer customers to self-sufficiency programs.
Advocate for the low-income population, giving priority to the elderly, disabled and families with young children 5 & under.
Refer potential applicants to Weatherization and other related programs.
Conduct assessments, provide services and referrals.
Provide information to clients on energy conservation, and refer customers for budgeting, nutrition, and related issues.
5.
Sends in documentation of assistance based on program requirements.
6.
Completes reports and monitors local budget based on program requirements.
7.
Coordinate with local resources through outreach to inform and be informed of available programs in the prospective service area.
8.
Maintains routine office procedures.
9.
Knowledgeably promotes and coordinates agency programs for the county.
Advertises and promotes all services throughout designated counties, educate the county of services available.
Knowledgeable of all program guidelines, updates, and changes as they are available.
Administer all programs correctly and knowledgeably.
10.
Assess and assists clients with energy assistance.
Ensuring that routine assessments and qualifications are conducted correctly.
Also making sure that clients are assisted with the appropriate category of funds to which they are eligible in a timely manner (often the same day) with correct documentation.
11.
Coordinates with county resources to maximize existing services to assist clients.
Also, coordinates with local resources to be knowledgeable of programs to assist clients.
Make correct referrals and advocate for clients based on the clients' assessment and the needs of the household with documentation of referrals.
Use all the perspective county resources to assist clients to become self-sufficient.
12.
Applications are downloaded into SHAH on a weekly basis and all documents are shredded once they have been attached to the SHAH software.
Payment listing must be emailed every Friday before you leave for the day.
13.
Attends requested staff meetings and conferences to enhance program knowledge.
Attend meetings/conferences as requested or as within staff members' control.
Ensures information is obtained from any missed meetings.
These meetings include but are not limited to face-to-face meetings as well as google hangout meetings.
14.
Assumes responsibility of any information disseminated through written correspondence, which includes but is not limited to email, mailed letters, hand delivered letters and performance improvement worksheets.
15.
Be on time and in the office every day as scheduled.
When you are going to be out, notify the immediate Supervisor or Executive Director 2 hours in advance.
16.
Limit outside distractions (visitors, sharing family/personal information with the customers and personal calls on agency phone and/ smart watches or cell phone).
17.
Use of internet for office purposes only:
no games, Facebook, etc.
during working hours.
18.
Assumes additional responsibilities as requested.
Additional tasks are completed thoroughly and within expectation.
19.
Any other duties as requested by Immediate Supervisor or Executive Director.
Program Design Assists Program Director with program development, quality, and evaluation.
Ensures compliance with TAC Rules Planning Keeps supervisor aware of program/service area needs to be addressed.
Participates in professional and performance development planning during annual evaluation with supervisor.
Assists in establishing written annual short-term and long-term goals and strategies for service areas.
Participates in planning meetings.
Communication 1.
Attends requested staff meetings and conferences to enhance program knowledge.
Meetings include, but are not limited to o Face to Face o Google Hangouts 2.
Attend meetings/conferences as requested or as within staff members' control.

Ensures information is obtained from any missed meetings.
4.
Reads all interoffice memorandum and maintains for future reference.
Asks for clarification if needed 5.
Communicates with supervisor regarding clients and services provided.
Keeps supervisor abreast of program issues in a timely manner and seeks supervisory guidance on a regular basis.

Models positive, open communication with staff.
7.
Cooperates with outside agencies and professionals as requested and keeps supervisor informed of all meetings, discussions, etc.

Recordkeeping and reporting Responsible for ensuring an efficient, confidential, centralized record-keeping of information.
Ensures that all necessary documentation is maintained.
Responsible for the quality of records kept.
Submits reports as requested Ongoing Monitoring Participates in monitoring process as assigned.
Completes assigned monitoring.
Self Assesment Participates in annual program self assessment process as assigned.
Human Resources 1.
Adheres to the standards of conduct.
Maintains personnel policy manual by replacing policies when updates are provided.
Maintains compliance with program and personnel policies/procedures and asks for guidance or clarification, if needed.
Coordinates job specific employee orientation with Director and/or designee.
Maintains a high level of confidentiality regarding employee personnel information and performance.
Fiscal Management Ensures accurate and timely completion of timesheets, purchase authorizations, and expense requests.
Completes reports and monitors local budget based on program requirements.
Other Assists with general office administrative duties (i.
e.
answering phones, greeting visitors, delivery and pickup of mail from other sites) as assigned.
Work Environment:
The Community Service Representative (CSR) worker performs work inside and outside in a variety of settings to include:
office and community environments with other GETCAP employees and clients.
The setting requires verbal and face-to-face contact with the community, clients, and other staff members.
The CSR performs most work during weekday hours.
Physical Requirements:
Occasional bending, stooping, or lifting up to 50 lbs.
